Learner Survey
Thank you for your participation in Project SHINE this semester.
In the following survey, we would like to hear about what you learned with your
SHINE tutor. Your answers will help us to improve our program. It will take
approximately 10 to 15 minutes to complete the survey.
You may complete this survey on a computer in English or on paper in
translation. If you have trouble reading or understanding the survey, please ask
for help.
If you would like to complete the survey online, click on “Go To Computer
Survey.” If the survey is hard to read because the size of the letters is too
small, you may make the letters larger by selecting “view” on your web browser
to increase the font size.If you would like to complete the survey on paper in English or translation,
click on the language you would like below. After you complete the survey,
please put it into an envelope, seal the envelope, and give it to your tutor,
teacher, or program staff person who will return it to Project SHINE. Your
answers will be confidential.
